Inpatient/outpatient Pharmacists (1099)

Pay rate : $70.00 hr (1099)

Duration: 1 year contract.

Location: Phoenix VA Health Care System (PVAHCS), located at: 650 E. Indian School Rd., Outpatient Pharmacy, building 8, Room A1215 Inpatient Pharmacy, Building 1, Room 3108 32nd Street Outpatient Pharmacy Phoenix, AZ 85012

Hours:

The facility operates 24 hours per day. Pharmacists are subject to varying work hours depending on staffing needs, which may be revised at the discretion of the Chief/Director of Pharmacy. Work hours may consist of the following 8, 11, or 12-hour schedules: 6:00am-2:30pm, 2:00pm-10:30pm. Overnight shifts 6:00pm-6:00am, and 6:00pm—6:00am may also be required pending staffing needs. Weekend and holiday coverage may also be required, based on staffing needs. Lunch breaks of 30 minutes, and other rest breaks, will be determined according to daily workload demands.

Description:

Services Provided : Shall provide OP/IP Pharmacists who can deliver a full spectrum of pharmacy services, in accordance with the specifications contained herein, to beneficiaries of the Department of Veterans Affairs (VA) Phoenix VA Health Care System (PVAHCS). 

Pharmacist Description of Services: Shall encompass the full range of pharmacy services equivalent to those provided in a state-of-the-art civilian medical treatment facility.

  • Shall dispense drugs prescribed by physicians and other health practitioners and provide patients with information regarding medications and their use. The pharmacist shall advise physicians and other health practitioners on the selection, dosages, interactions, and side effects of medications. The pharmacist must be able to evaluate provider medication orders for pharmacologic duplications, incompatibilities, side effects, drug–food and drug–drug interactions, and allergies.
  • The Pharmacist must be able to appropriately address potential therapeutic problems such as inappropriate dosage, incorrect strength, or improper dosing intervals. The pharmacist shall effectively communicate questionable therapies to providers and recommend reasonable alternatives in accordance with formulary guidelines established in the Computerized Patient Record System (CPRS).
  • The Pharmacist shall counsel patients and respond to questions regarding prescription drugs, including possible adverse reactions or interactions. The pharmacist shall also provide advice on durable medical equipment and home healthcare supplies.
  • The Pharmacist must demonstrate positive interpersonal skills when interacting with providers, patients, other Medical Center employees, and fellow pharmacy staff members in order to foster an atmosphere of care, helpfulness, and cooperation.
  • The Pharmacist shall ensure that all medication orders are entered and verified in the electronic patient medical record (VistA).
  • The Pharmacist shall ensure compliance with all state and federal regulations governing the receipt, safeguarding, and issuance of controlled substances.
  • The Pharmacist shall provide strict security for all drugs and controlled substances by maintaining accurate records and perpetual inventory control.

QUALIFICATIONS:

  • Shall have a full, current and unrestricted license in any State, Territory, or Commonwealth of the United States or the District of Columbia) when services are performed onsite on VA property. Must be licensed for controlled substances, with qualifications to practice in any states. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. Contractor's pharmacist (s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract.
  • Pharmacists must be graduates of an Accreditation Council for Pharmacy Education (ACPE)-accredited College or School of Pharmacy with a baccalaureate degree in Pharmacy (B.S. Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
