Case Management Coordinator (Remote / Miami-Dade Area)
Payrate: 34.14 an hour
Location: Work From Home (Candidates must reside within North Miami, Aventura, Hialeah, or Miramar , FL)
Travel Required: ≈ 50–75% local travel to member homes, assisted living facilities, and nursing homes within the region.

We are seeking a self-motivated, energetic, detail-oriented, highly organized, tech-savvy Case Management Coordinator to join our Case Management team. This role involves coordinating and managing case activities telephonically and face-to-face to evaluate members’ medical needs and promote overall wellness.
